Case Management

A service that.....

  • Promotes maximum independence and successful integration into community living for individuals with developmental disabilities.
  • Minimizes individual reliance on institutional services, and
  • Maintains accountability and continuity of services to individuals and families as long as services are required. 

Services include......

  • Identifying an individual's needs and preferred lifestyle,
  • Creating a Person-Centered Support Plan, arranging for and securing supports outlined in the plan, and monitoring of the plan to ensure effective implementation,
  • Developing and accessing natural and generic community supports and funding options,
  • Ensuring the rights of individuals are respected and exercised,
  • Providing advocacy, including pursuing means for gaining access to needed services and entitlements,
  • Seeking modification of service systems when necessary to increase the accessibility to those systems by the individual, and
  • Planning for the transition of services from one setting to another.

Case management services can be obtained from a licensed Community Service Provider, case management agency, or an individual who is licensed to provide case management services.  Case managers must maintain compliance with laws and regulations, satisfy any licensing requirements, and affiliate with the local CDDO.
